Consultation (Urology, Male Fertility, Sexual Medicine)
If you have a PPO insurance, you will be responsible for you specialist co-pay (as determined by your insurance, usually listed on your insurance card).
We accept Medicare. Your out-of-pocket costs will depend on your specific plan coverage. We do not accept Medicare Advantage plans unless they are PPO.
For patients with EPO, HMO, or self-pay, the cost of the initial consultation is $250.
For patients in California, initial telemedicine consultations are available.
All initial vasectomy reversal consultations are FREE.
Low Testosterone

Vasectomy
Insurance coverage for vasectomy can be unpredictable, with patients sometimes facing unexpected out-of-pocket costs due to variable reimbursement. To avoid this uncertainty, we’ve set a clear procedure fee of $600. This transparent approach means you’ll know exactly what to expect, with no surprises after your procedure.
To verify the success of your vasectomy, purchase of a mail-in post-vasectomy semen analysis (PVSA) is $150.
If a patient wishes to be sent to a laboratory through insurance for a PVSA, a semen analysis lab requisition will be given to a patient. From our experience, commercial lab testing for semen analyses in the Los Angeles area can be rather difficult.
To better understand of what your out-of-pocket costs would be with your insurance plan, follow the exact directions below. If your out of pocket fee is lower than our cash price, reach out to our office with a good faith estimate provided by your insurance company, and we will do our best to match.
The best way to know your exact cost is to:
Call your insurance provider and ask if vasectomy (CPT code 55250) is covered under your plan. If diagnosis code is requested, it is most commonly Z30.2 (Encounter for sterilization).
Confirm whether the procedure is subject to a deductible, co-pay, or coinsurance. Ask them directly what you should expect your out of pocket costs will be.
We will provide you with a superbill and clinical notes that you can provide your insurance for reimbursement.
Penile Girth Enhancement
Until October 2025, initial consultations are free. We offer telemedicine initial consultations as well. We welcome patients from all across the United States, as well as international as well.
