Overview

What is Sperm Retrieval?

Sperm retrieval refers to specialized procedures used to obtain sperm directly from the testicle or epididymis when sperm is not present in the ejaculate. This is used to address very low sperm counts, reproductive tract blockages, prior vasectomy, or congenital absence of the vas deferens.

Retrieved sperm is used in conjunction with assisted reproductive technologies like IVF or ICSI, where even minimal sperm quantities can enable successful fertilization in the laboratory.

Procedures

Retrieval Techniques

Micro-TESE (Microdissection Testicular Sperm Extraction)

An advanced surgical technique performed under high-powered magnification to locate areas of the testicle with active sperm production. Particularly effective for severe non-obstructive azoospermia. Maximizes retrieval success while minimizing removal of unnecessary tissue.

TESE (Testicular Sperm Extraction)

Involves removal of small amounts of testicular tissue to retrieve viable sperm. Extracted sperm can be used for ICSI in combination with IVF.

PESA (Percutaneous Epididymal Sperm Aspiration)

Minimally invasive — aspirates sperm directly from the epididymis using a fine needle. Commonly used in obstructive azoospermia (e.g., after vasectomy). Quick procedure with minimal recovery time.

TESA (Testicular Sperm Aspiration)

A fine needle aspirates sperm directly from the testicles. Often used with IVF or as part of a diagnostic evaluation. Well-tolerated and typically performed under local anesthesia.

FAQs

Frequently Asked Questions

Is sperm retrieval painful?

No — the procedure is performed under local or general anesthesia. Postoperative discomfort is mild and manageable with over-the-counter medications and ice.

How long does the procedure take?

Depending on the technique, anywhere from 30 minutes to 2 hours. Timing will be discussed during your consultation.

Can retrieved sperm be used for natural conception?

No — sperm retrieved surgically must be used with assisted reproductive technologies such as IVF or ICSI.

How is sperm preserved after retrieval?

Sperm not used immediately can be cryopreserved (frozen) for future use — it can remain viable indefinitely in proper storage conditions.

What are the success rates?

Success varies by cause and technique. Micro-TESE has a success rate of up to 50% for non-obstructive azoospermia — among the best available.

What if no sperm is found?

Alternative options such as donor sperm or other fertility treatments will be discussed if no sperm is retrieved.

Can sperm be retrieved after a vasectomy?

Yes — PESA and TESA are particularly effective for men with obstructive azoospermia following vasectomy.

Are there risks?

Risks are minimal and may include temporary swelling, bruising, or discomfort. Serious complications are extremely rare.
